Set a google alert for your name and address. Then you’ll get an email whenever a new site posts your name and address. You can catch each new instance and request a takedown.
I use Google Alerts quite a bit for those things I want an alert on when they occur at very sporadic times. It can be used for news items, copyrighted material or media needing attribution, etc.
